Baptism Registration Form

You can use this online form to share with us your desire to be baptized. Your completed form will be emailed to the church office and a staff member will contact you. We ask that every person who desires to be baptized complete this form.

Be sure to also download a copy of the Baptism Information Sheet. You can also click on the link to the left. We believe you having this information is an important step in your choosing to follow Christ.
